In Hospital Care
Currently we have contracts with Blount Memorial, Mercy Health Partners and
Jefferson Memorial Hospitals in the Knoxville area. If your hospital has a contract with us, obtain an order for “Ostomy Nurse Consultation,” and then contact us as soon as possible so we can coordinate teaching sessions with the patient and their caregivers. We can also see patients who have had their ostomy prior to admission if they are having problems. We will visit the patient and family in the hospital and teach basic care of their ostomy as well as provide written literature, contact information and how to get their supplies. Our nurses can also be consulted for difficult wounds/fistula management.

Home Health/Hospice
While on home health, the agency must provide supplies for the patient’s ostomy even if the patient is being seen for a reason other than their ostomy. For hospice patients, this may not apply. Please call us. Insurance exceptions are Blue Cross/Blue Shield and Blue Care (we can supply products to these patients.) If unsure, call. If you are having problems with an ostomy patient we may be able to troubleshoot by phone. If necessary, the patient can be seen in our clinic for a $25 fee, which can be paid by the patient or your agency.
An appointment is necessary and we need to arrange for supplies if changes are necessary. Prior to discharge please assist the patient in arranging for supplies.

Pre-Op or Before Surgery
Our ostomy nurses will mark stoma site and begin counseling with patient on basic care and what to expect life to be like with an ostomy. This session is a free service at The Pouch Place, Inc.

Skilled Care/Nursing Home
If the patient is “skilled” the facility is responsible for providing supplies. If there are problems with an ostomy patient the nursing staff can call us and we can troubleshoot by phone. If problems persist we can come to your facility for a fee. We need a doctor’s order and approval from the Director of
Nursing which may be faxed to us. Prior to discharging a patient home, it is a good idea to assist the patient in arranging for supplies. If the patient is not “skilled” and resides in a nursing home we can provide supplies just like they are living at home provided we take their insurance.

Supplies
To arrange for supplies, go to the Forms page. Print off the Enrollment Form. Complete this and send or fax this along with a copy of the front and back of all their insurance cards to us. If you do not have access to a computer, we can send or fax the required forms and you can mail them back to us with a copy of the front and back of the insurance cards. We accept Medicare, BC/BS, Humana, Blue Care, Cigna and most private insurances. We cannot ship supplies to a patient until we have the required forms in hand. If you plan to purchase supplies and not have us file insurance we will not need as much information.  Contact us for pricing.  WE ARE NOT IN NETWORK WITH UNITED HEALTH CARE OR AETNA.
