In Hospital Care

In HospitalWe currently have contracts in place with Tennova Medical Centers (Physicians Plaza, Turkey Creek & North). If your hospital has a contract with us, obtain an order for “Ostomy Nurse Consultation,” and then contact us as soon as possible so we can coordinate teaching sessions with the patient and their caregivers. We can also see patients who have had their ostomy prior to admission if they are having problems. We will visit the patient and family in the hospital and teach basic care of their ostomy as well as provide written literature, contact information and how to get their supplies. Our nurses can also be consulted for difficult wounds / fistula management.

Home Health / Hospice

Home HealthWhile on home health or hospice, the agency must provide ostomy supplies for the patient on Medicare even if they are being seen for a reason other than their ostomy. Private insurances vary on their rules for Home Healthcare, please check with us. If you are having problems with an ostomy patient we may be able to troubleshoot by phone. If necessary, the patient can be seen in our clinic for a $30 fee, which can be paid by the patient or your agency. An appointment is necessary and we need to arrange for supplies if changes are necessary. Prior to discharge please assist the patient in arranging for supplies.

Pre-Op or Before Surgery

SurgeryOur ostomy nurses are available to answer any questions about pre-operative preparations, such as devices , post-operative issues, stoma care and any other concerns the patient may have. We will mark stoma site and begin counseling with patient on basic care and what to expect life to be like with an ostomy. This pre-operative session is a free service offered by The Pouch Place and does require an appointment.

Skilled Care / Nursing Home

Skilled CareIf the Medicare patient is “skilled” the facility is responsible for providing supplies. Other insurances may have difference rules, please check with us. If there are problems with an ostomy patient the nursing staff can call us and we can troubleshoot by phone. If problems persist we can come to your facility for a fee. We need a doctor’s order and approval from the Director of Nursing which may be faxed to us. Prior to discharging a patient home, it is a good idea to assist the patient in arranging for supplies. If the patient is not “skilled” and resides in a nursing home, we can provide supplies just like they are living at home provided we take their insurance.
